Legacy Hour-by Hour Care at your Advantage

We understand that every client and their needs are different. Legacy Health Advantage recognizes that caring for family or loved ones can be challenging. We truly encourage family respite/relief time. Therefore, we offer the amazing service of hourly home care services.

 

This gives you the advantage to choose any homecare service on an hourly as-needed basis. Whether it be companionship, salon transport, shopping, appointments, gardening, or simply ensuring your loved one's safety, we are here for you.

Legacy Live-In Home Care

Our live-in homecare service is a peaceful advantage to ensure your loved one receives around the clock, safe, wholesome care in a familiar environment.  It is consistent worry-free 24 hr care utilizing 2 experienced caregivers rotating shifts of 12 hrs, to ensure the highest quality of care. As experienced health care providers, we have seen one of the best ways to improve quality of life is simply remaining in a familiar environment which more than often is one’s Home.

A strategic, well - organized scheduled is developed by our expert care team to meet the goals of the client, their family, and providers. Our care team is always willing to compassionately cook a favorite meal and/or assist with eating while they promote independence, maintain a clean peaceful atmosphere, provide private personal care, safely manage medications, and/or take a load off your hands by running errands as needed.

This is why LHA has a great facility companion service designed to ensure your loved one feels comforted by a familiar face and VIP care. Our personal care companions are patient-driven advocates. Our team is well trained to ensure a safe environment is maintained and accurate reporting of any problems to the appropriate facility staff members.

Legacy Companion Care

Companion care also falls under the titles of personal care assistants and homemaker services. Legacy Healthcare Advantage compassionate companion care is a nurturing service for clients. The benefits of a companion are to provide emotional support, companionship and promote client independence while at home.

 

This service also includes a variety of care services that support those within their golden ages live more comfortable . These services can include light housekeeping, assistance with activities of daily living (ADLs), medication reminders and much, much more. Companion care can be customized for those in their homes, assisted living, nursing homes, senior living or healthcare facilities.

Legacy Facility to Home transition care

Many times, one is discharged from the hospital or healthcare rehab they will have restraints to daily activities. The transition care services are care specialized based on your discharge goals. We ensure health providers orders are followed, clients remain safe and your needs are met through this transition.

 

An example plan for this care may consist of ROM of motion, medication reminders, assistance with light cleaning or lifting, meal preparation, doctor’s appointment transportation, and many more. This service is designed to ease your worry regarding your limited capabilities and provide client-centered safe care.
